fix: resolve dialyzer warnings and restore 3-tuple support

- Update function specs to accurately reflect 3-tuple returns
- Fix error normalization from strings to Error structs
- Restore directive handling in chain execution
- Fix GitHub Issues HTTP response handling
- Fix Weather tool System.fetch_env usage
- Remove unreachable code patterns
- Add targeted dialyzer suppressions for tool limitations

All tests pass (312/312) and dialyzer reports 0 errors.
