docs: split examples into dedicated files (#23)

* docs: split examples into dedicated files

* fix: address code review feedback on example docs and tests

Use fully-qualified gRPC method names in URI rewrite and upstream
forwarding docs/comments, switch embed directive to glob pattern,
clarify combined config comment, and make upstream tests hermetic
by using a local echo server container instead of external service.

Replace external grpc-echo.semior.dev dependency in TestMain_Examples
with local container to keep CI hermetic. Also apply linter suggestions
(maps.Copy, range over int).
