feature(roles): implement User Roles API feature

Main requirements for completing this task...

- write unit tests for Roles API endpoints
- add sample data for tests
- setup migrations and seeders
- setup models
- setup model-service classes
- setup controllers and middleware
- set default role for new User
- add User's role to access token
- document the API with Swagger
- implement PR review feedback
- add authorisation to existing routes

Other minor updates included...

- refactor(users): change middleware naming... According to feedback from TTL
- chore(app): add a script to reset migrations
- refactor(app): move sequelize cli to devDep...
- refactor(app): set mocha to run tests recursively
- bug(passwords): make test hooks asyncronous
