docs(release): add v1.4.0 notes, fix typos, add changelog Unreleased section

Add v1.4.0 section to RELEASE_NOTES.md with migration table. Fix v2.1.0
typos in migration guide (should be v1.1.0). Add Unreleased section and
comparison links to CHANGELOG.md per keepachangelog spec.
