Fix #48 - Add more utterances

* Add a new custom slot type TRANSIT_MODES to avoid adding all the combination of utterances for all GTFS route types.  Instead, we just add "where is my {TransitMode}" to utterances, and add the transit modes to` /interaction model/customSlotTypes/TRANSIT_MODES`.
* Update Readme for creating custom slot type when setting up skill
