Simplify generated Fortran indexing and presence query formatting

Use one-based prefix sections directly in partial setters and remove the
lower/upper-bound temporaries and generation context. Wrap long assignments
to stay within Fortran's source line limit.

Change idx_check to accept a shape vector and emit shape(this%field) calls.
Declare shape as a local intrinsic so host-associated constants with the
same name remain supported.

Restore compact presence checks, remove excess whitespace around query
loops, document the indexing contract, and regenerate examples and fixtures.
Cover rebased setter arguments, invalid indices, empty extents, long field
names, and intrinsic-name collisions in runtime regression tests.

Validation: 415 Python tests, Ruff, strict mypy, nine generated-output
checks, native Fortran conformance/examples, and 14 f2py integration tests.
