Harden Bash access to credential files with a PreToolUse hook

Wire block-env-read.sh as a PreToolUse Bash hook and broaden the Read deny globs to nested dotenv files. The hook denies Bash commands that name a credential file or run a recursive scan that could traverse into one, closing a gap the Read-only deny rules did not cover (a recursive scan can read a protected file without ever naming it).
