Store reprocessing summary in a dedicated upload and searchable entry (#6)

- Disambiguate the two upload fields: rename `upload_ids` → `target_upload_ids`,
  and make `upload_id` optional + auto-filled (was required) — it stays the
  per-upload association so runs show in the upload's action history.
- Write each run's summary to a dedicated per-user "Reprocessing summaries"
  upload (permanent, unlike the pruned action-instance storage) instead of
  reusing the context `upload_id`, and record it as a searchable entry
  (`references` = reprocessed upload ids) so runs are queryable by which
  uploads they touched.
