ISSUE-248 - Added field diseaseFromSource to check for duplicates Added the field ``diseaseFromSource`` to the ones used to check for duplicates, solving the issue of non-duplicated evidence strings being considered as so when field ``diseaseFromSourceMappedId`` was missing. Also updated the documentation.
