Cleanup namespaces on output

When serializing out, run through ElementTree with all expected
namespaces registered, so that namespaces are cleaned up and declared
at the top of the html document.

This affects other aspects of the output HTML, specifically, attributes
are now sorted by name, XML comments are removed, empty tags are
expanded, and unicode is output explicitly (not as escaped code-points)

This includes bumping travis python3 version to 3.5, since something I
did for 2/3 compatability seems to break on 3.3.
