fix: correct pointer, float, string, and literal miscompiles (#393)

Pointer subtraction is ptrdiff_t. Ordered floating compares treat NaN as
unordered. Unsigned 64-bit and __int128 conversions to float and double
round once. va_start skips named stack arguments. Wide literals keep their
prefix, decode as code units, and initialize matching arrays. Character
constants sign-extend. Oversized hex and octal literals stay unsigned
__int128.
