opt: strength-reduce simple induction variables (#380)

After LICM, rewrite a multiply of a simple induction variable by a
loop-invariant factor into a pinned running total. A constant-stride
address of an 8-byte induction variable becomes a pointer that steps
by the stride. Narrow indexes stay in the loop. A same-value pointer
add updates the register that already holds the pointer.

Git CC=trans -O2 prove: 1056 files / 33706 tests.
