Add a CHANGELOG and the sources left untracked by the previous commit

The refactoring commit only recorded the modified files: the eighteen
classes it introduced (BerTlvInputStream, the new model objects and their
enums, CardIdentificationUtils, TagValueDecoder) and their tests stayed
untracked, so the tree as pushed did not compile.

Also ignore the Eclipse and macOS files, and point the README at 3.2.0.
