refactor(codegen): replace OOP quadruples with procedure-scoped data IR

Lower AST to IntermediateRepresentation (procedures with frames and flat
bodies), run seal/jump-to-next passes, and emit from Op switches. Remove the
quadruples class hierarchy and generateCode dispatch. Add IR dump goldens and
pass unit tests.
