docs: agent image has moved to ghcr.io/nvidia/nodewright/agent (#385)

The agent image path migration is done, but the docs and the operator's
own default still pointed at the pre-rename path. Released agent tags up
to `v6.4.1` are on `ghcr.io/nvidia/skyhook/agent`; `v6.4.2` and later
publish to `ghcr.io/nvidia/nodewright/agent`, because `agent-ci.yaml`
derives `IMAGE_NAME` from `github.repository`, which the repository
rename changed.

Docs and comments:
- README.md, chart/README.md, docs/release-process.md: drop the
  "migration pending" wording and name the new path.
- .claude/CLAUDE.md: move the agent image from the pending list to the
  already-migrated list. The namespace (`skyhook`) is still pending.
- chart/values.yaml: the pinned digest comment claimed the digest lives
  on `skyhook/agent:v6.4.2`. It does not; that manifest 404s there and
  resolves on `nodewright/agent`. The pinned digest value was already
  correct, only the comment was wrong.

Defaults and tooling:
- operator/internal/controller/skyhook_controller.go: the `AGENT_IMAGE`
  env default pointed at a path that no longer receives releases. The
  pre-existing TODO is kept and narrowed, since pinning a released
  version instead of `:latest` is still outstanding.
- operator/config/manager/manager.yaml: kustomize had drifted from the
  chart, which already used the new path.
- operator/Makefile, agent/Makefile, .vscode/launch.json: build and dev
  tooling defaults.

The `agentless` mock image is a separate path and is not touched here.
