fix: Ignore BOM and reject malformed UTF-8

Ignored includes UnicodeBOM (U+FEFF), which was rejected everywhere.
SourceCharacter is any Unicode scalar value, so strings and comments
must reject bytes encoding none, including surrogates and
values above U+10FFFF.
