blog: Part 11 - counted loops

Cover step F17 (DO LOOP +LOOP I J LEAVE UNLOOP through both backends,
loop parameters on the return stack) as the next diary entry, centered
on the nested-loop stack-effect bug the checker had carried since it
first modeled DO...LOOP.

Add tight orgit anchors for the transcluded regions (I/J/LEAVE/UNLOOP
handling and the structural DO loop in eval_direct.hpp, the LEAVE
back-patch in codegen.hpp, +LOOP's boundary-crossing termination in
vm.hpp, and the corrected DO effect model in stack_effect.hpp);
verified with g++-16 -fsyntax-only. Update Part 10's next-link and
index.org, and regenerate the changed posts' markdown via make blog-md.
