feat(ast): implement do-while loops

Wire grammar productions already present for do/while into a
DoWhileLoopHeader with body-before-test and continue-before-condition
semantics via LoopHeader virtuals (no dynamic_cast). Functional tests
cover once-through, multi-iteration, break, and continue.
