fix: harden deployment cancellation classification and observability

- Classify Node AbortError rejections (fs, streams) as cancellations in
  processing metrics, unwrapping the deadline reason from their cause
- Record validation failures as a distinct client-error outcome so
  outcome="error" only reflects server-side problems
- Log genuine failures that race with a disconnect or the processing
  deadline before returning the 499/408 cancellation response
- Document the 499 response for POST /entities
- Rename getConcurrency to getPositiveInteger since it also reads the
  processing timeout
- Document the single-signal memoization assumption in toDeploymentFile
- Align the worlds-manager mock deployer casing with the real adapter
- Give the paced-upload entity-deployer test hook a 30s budget to avoid
  flaking under parallel-worker load
