refactor(core): optimize HeavyKeeper concurrency and add tests

- Add concurrency documentation for HeavyKeeper
- Improve BufferedCounter and HeavyKeeper implementations
- Enhance cache sync listener and worker listener
- Add comprehensive unit tests for key components
