feat: dashboard data-capture (segments + NDJSON contract + gate + refresh) (#98)

- add internal/dashboard: Context stdin schema, typed NDJSON fragment
  contract (tile/bar/group/note/html), fail-open Parse, built-in git/roadmap
  segment providers, and config helpers (defaults, cooldown, enable gate)
- add `ouroboros dashboard segment <name>` and `ouroboros dashboard refresh`
  CLI commands; refresh is gated by dashboard.enabled (default off), honors
  a cooldown, and writes collected NDJSON to disk
- exec/shell segment producers are declared in config but not yet executed
  this slice (recorded as a drop) -- no HTML render, no Stop-hook, no age-out
- document the new package and CLI ops in CLAUDE.md
