Minor edits to QuantumProgram docs and repr (#3006)

### Summary
This PR:
- Adds missing docs for `meas_type` to the init of quantum program
- Fixes the `repr` of CircuitItem (formerly QuantumProgramSimpleItem)
and SamplexItem (previously QuantumProgramSamplexItem)
