Consolidate per-stream stdout/stderr config into StreamConfig

Replace the four parallel stdout_*/stderr_* field pairs across Command,
Spawned and RunningProcess with a single per-stream StreamConfig
(encoding/handler/tee/terminator), and thread it whole through every
pump_lines_core call site instead of unpacking each field individually.
Public Command API is unchanged.
