memtx: fix MVCC tuple clarifying for DML statements

DELETE, UPDATE, and UPSERT need to see prepared, but unconfirmed
tuples while building a statement, otherwise they may clarify a wrong
tuple under read-confirmed isolation. Split point lookup paths so write
statements use prepared-visible lookup, while read-only lookups keep
normal read visibility.
