Simplify redundant comments in indice/risk/concurrency/storage

Phase 9 of the codebase-wide comment-simplification effort. Remove
section banners, WHAT-style comments that restate the declaration below
them, and one duplicated Meyers-singleton note. Comment-only; no code,
headers, license text, or Machinist blocks are touched.

- globals.cpp: drop the duplicate Meyers-singleton note (keep one) and
  the section divider before the fixings store.
- splat.cpp: drop the "helper functions for UnSplat" banner.
- threadpool.hpp: drop the singleton restatement and the ActiveWait
  docblock; Savine/Andreasen license header preserved verbatim.
- report.cpp: drop the SetAll restatement; Machinist block preserved.
- _repository.hpp: drop per-method WHAT labels that restate each
  signature.
