style: fix test file cosmetics (Priority 8) and update SKILL.md (G1)

- 8a: Rename 34 test names from suffix to prefix style across 5 files
  (e.g., DefaultConstructionTest -> TestDefaultConstruction)
- 8b: Move #include <gtest/gtest.h> to first include after file header
  in 15 test files that had it out of order
- 8c: Add missing EOF newlines to 42 test files
- 8d: Add missing file header to test_daybasis.cpp, fix 5 date formats
  to YYYY/M/D convention
- 8e: Fix typos in test_algorithms.cpp (Uniary->Unary, OP->Op) and
  test_functionals.cpp (FunctionlsTest->FunctionalsTest)
- 8f: Remove stray backslash from include in test_environment.cpp
- G1: Add whole-codebase scan instructions to dal-code-style-review
  SKILL.md for when args include 'whole code base' or 'entire codebase'

All 753 tests pass.
