Tidy calibration source and Phase A doc references

Cosmetic-only pass, no behavior change:
- calibration.cpp: rename locals ret_val -> retval (DAL trailing-underscore
  convention reserves the _ suffix for members; ret_val read as snake_case
  without it) and expand the TapeGuard_ destructor try/catch onto multiple
  lines for readability.
- ycinstrument.cpp: drop the redundant ===== banner comment lines around the
  Phase A templated-rate section header.
- aad-analytic-jacobian-curve-calibration.md: replace brittle line-number
  citations (calibration.cpp:221-235, :375-377) with symbol-name references
  (YieldCurveCalibrationFunc_::F, the Underdetermined::Find call in
  CalibrateYieldCurve) so the doc survives source reformatting.
