2.6.0: fix #119/#120 follow-up — allele audit denominator + categorical/empty-input hardening (#126)

- Position-specific novel-allele denominator + assigned-allele context columns; pct clamped to [0,1]; shared scan-window constant.
- observed=True on every column-based groupby (AnnData/H5AD categorical crash class).
- annotate_combined_df empty-input fix; de-xfail.
- Key/index-based aggregation alignment in til/clonotype (drop positional .values).
- Test cleanup: un-shadow duplicate class, unused imports, import sorting.
