feat: add alerts API endpoint for grouped helix.error events

GET /api/v1/notifications/alerts?instrument_id=X returns helix.error
events from the last 7 days grouped by metadata fingerprint, with
occurrence count, first/last seen, and up to 5 example entity IDs.
