fix(api): return 503 within 8s when DB pool is saturated

Adds a context deadline to the monitor bins query so the endpoint fails
fast with 503 instead of hanging until the client disconnects (30s).
Also adds migration 009 with a composite index on
(instrument_id, created_at DESC) to speed up the /monitor/bins query.
