feat(monitor): show server version for registries and queries

Read the Nanopub-Registry-Version header for registries and the
forthcoming Nanopub-Query-Version header for queries, and surface them
as a new Version column in the main table and CSV export. Wiring up
the query header now means the column will populate automatically once
nanopub-query starts emitting it.
