feat: release planning, version filtering, and gate verification (v0.3.0)

Add rtmx release command family for requirements-driven tagged releases:
- rtmx release assign/unassign: target requirements to a version
- rtmx release scope: planning summary with effort and blockers
- rtmx release gate: closed-loop verification (exit 0/1, --verify, --json)

Add --version flag to status, backlog, and verify for version-scoped views.
Add TargetVersion filtering to database.FilterOptions with ByVersion grouping.
Auto-set started_date/completed_date on verify status transitions.

Add pre-push hook enforcing release gate on version tag pushes.
Codify semver versioning policy in CLAUDE.md defining major/minor/patch
bump criteria against the CLI public surface.

Add REQ-PLAN-001 through REQ-PLAN-013 for release planning, identity,
velocity, and attribution. Add REQ-PLUGIN-005 through REQ-PLUGIN-007
for schema plugin framework.
