limbo: remove acks from its public API

This allows to finally untie the relay and limbo modules from each
other.

It also lets the callers not care what is considered an ack - an
LSN assignment, a replica ack?

And additionally - it encapsulates more of the limbo logic, which
will be quite handy in the future commits.
