feat: render image content blocks as [image: <mime>, <N> bytes] markers

Pasted screenshots and tool-returned images in user content previously hit
the catch-all warning path and were dropped entirely. Extract them as a
short placeholder instead so search and dump output at least indicate an
image was present, and URL-sourced images surface the URL for context.
