Add implementation plan section to PR template (#4737)

AI-planned PRs currently lose the approved plan after the conversation
ends. Adding a collapsible "Implementation plan" section to the PR
template ensures reviewers can see the intended design and tradeoffs
for any AI-assisted PR, regardless of which workflow created it.
