Add skills system architecture documentation (#4758)

The skills subsystem has been fully implemented but had no architecture
documentation. Add docs/arch/12-skills-system.md covering the complete
skill lifecycle, OCI distribution, git resolution, security model, and
toolhive-core dependency. Cross-reference from the README index, core
concepts (Skill added as a noun), registry system, and groups docs.
