feat: add --all-projects and --project-regexp flags to search

Search across multiple projects at once:
- `--all-projects`: search ALL projects under ~/.claude/projects/
- `-P REGEXP` / `--project-regexp REGEXP`: filter projects by path regexp (repeatable)

When multiple projects are searched, results are grouped under colored project headers
(inspired by ripgrep's file grouping). The --project path flag is used only in
single-project mode (existing default behavior is unchanged).

Also adds `projectPath` field to JSON output in multi-project mode and respects
--max-results as a global limit across all projects.
