Fix RETURNING scan triggering AfterScanHook and add integration tests

- Add scanValueReturning() that skips AfterScanHook for INSERT/REPLACE
  RETURNING scans, preserving expected hook ordering
- Add comprehensive integration tests for all new features:
  IsDuplicate, ILike, typed slice IN, Incr/Decr, SetRaw, FOR UPDATE
  SKIP LOCKED/NOWAIT, RETURNING, subqueries, Raw WHERE, RunQueryT
