fix: use record author instead of repo owner in GitHub shim responses (#157)

Issues, comments, PRs, and reviews now show the actual DWN record author
in the `user` field instead of always showing the repository owner.
The `merged_by` field reads the mergedBy DID from the merge result data.
The `author_association` is dynamically set based on whether the author
matches the repo owner.

Closes #120
