Add README.md support and enforce hyphenated tags

- Support both file patterns: content/2025/slug.md and content/2025/slug/README.md
- Both patterns build to dist/2025/slug/index.html
- Detect and error on conflicting file patterns (both exist for same slug)
- Enforce hyphenated tags: reject tags with spaces, require hyphens
- Update documentation with examples and warnings
- Update test fixtures to use hyphenated tags
- Update tests to expect hyphenated tags
