fix(codacy): remove unused imports and fix lexical declarations

- Remove unused BASE64_CHARSET import from entropy-normalizer.js
- Remove unused parseTemplate import from custom.js
- Remove duplicate generateTemplatePassword imports from index.js
- Wrap case block const declarations in braces (service.js lines 214-217)
- Apply prettier formatting to affected files

All 1853 tests passing.
