fix(codacy): resolve 194 static analysis issues

- Remove unused CHARACTER_SET_METADATA import from PasswordBuilder.js
- Apply prettier formatting to core domain and generator files
- Fix camelCase naming: generated_at → generatedAt in output-formatter.js
- Fix camelCase naming: online_throttled → onlineThrottled etc in analyzer
- Update tests to match new camelCase property names
- Format WebUIController.js and PasswordViewModel.js

All 1853 tests passing.
