Upstream Token Swap Middleware (#3606)

Adds a new middleware component that enables MCP servers to receive upstream IdP tokens when using ToolHive's embedded auth server.

When clients authenticate with ToolHive's embedded auth server, the system now automatically exchanges ToolHive-issued JWTs for the original upstream IdP tokens (from GitHub, Google, etc.) before forwarding requests to MCP servers.
