Remove redundant comments that describe what code obviously does

Kept comments that explain WHY (algorithm choices, ownership, lifecycle)
or document non-obvious behavior. Removed comments that:
- Duplicate what can be inferred from function names
- Describe obvious operations like "build X from Y"
- Comment function clause patterns that are self-documenting
