refactor alb mechanisms (#903)

* refactor tsm, nlm, fr, and fgr mechanisms to use new CaptureResponseWriter
* bugfix: regression - expand '*' path wildcard
* restore "stack" field to all log entries
* add Pool() and HealthyTargets() funcs to Mechanism interface
* respect timeout configs and update defaults
