Add index resolution to DalvikBytecodeOperation type

The public DalvikBytecodeOperation type now correctly reflects that
index fields are resolved to their actual types:
- methodIndex → method: DalvikExecutableMethod
- fieldIndex → field: DalvikExecutableField
- typeIndex → type: string
- stringIndex → string: string

Add type tests to verify these resolved fields exist with correct types.

Also fix typo: IndexIntoFieldIds → IndexIntoStringIds in the stringIndex
check of IndexResolvedOperation.
