Convert DEX addresses from code units to instruction indices

Change the representation of addresses in try blocks, catch handlers, and
branch instructions from 16-bit code unit offsets to instruction indices.

- startAddress and instructionCount in tries now use instruction indices
- address and catchAllAddress in handlers now use instruction indices
- branchOffset in instructions now uses instruction-relative offsets
- Debug info addressDiff is kept as code units for VM compatibility

Binary compatibility is maintained through conversion during parse/unparse.
