feat: Simplify CLI with unified setup command and add health/diff commands

Major changes:
- Add `rtmx setup` command that combines init, install, makefile, bootstrap
- Add --branch and --pr flags for git-isolated setup workflow
- Add `rtmx health` command for CI/CD integration checks
- Add `rtmx diff` command for RTM comparison in PRs
- Remove redundant commands: init, install, makefile, bootstrap, integrate
- Add GitHub Actions workflows for RTM validation

Other improvements:
- Switch license from MIT to Apache 2.0
- Rewrite README with terminal output examples
- Add NOTICE file per Apache 2.0 requirements
- Update contact to ioTACTICAL LLC

Command count reduced from 17 to 12.
