* cover loading multiple batches in preview and import

* rm comments with log statements

* divide code into commented sections

* fix off-by-one error

* make sure dbRows is not null

* add log statements for debugging

* sync changes in preview and import

* rm additional preloading of batches

* simplify progress tracking

* replace preview reader with import reader

* replace with ref to empty list

* refactor construction of header into one-liner

* switch to lazy eval of logging parameter

* rename boolean end for clarity

* make parameters final

* fix parameter in error log

* rm IOException from method signature as it is never thrown

* switch to 0-based logging for batches
