Fix remaining linting issues

- Remove unused extractRepoFromError function
- Fix SA4022: address comparison in table test (check removed)
- Fix SA9003: remove empty branches in cached_fetcher.go and fetch.go
- Fix SA4000: remove duplicate condition in refresh scheduler test
- Clean up unused skippedRepos variable
- All staticcheck issues resolved, linter now passes
